Life Insurance Claims

To file a claim on a life insurance policy issued by American General Life and Accident, complete the appropriate claim forms listed below.

Death Claim

Waiver of Premium Claim

• Waiver of Premium--(Form 182) The form must be completed in full by the insured and the treating physician certifying disability. Please, include all policy numbers to be considered on the form. It along with the HIPAA form should be submitted to the address in the upper left corner of the form

Specific Loss Claim

• Specific Loss (Form 183)
  This form is to be completed by the insured/claimant and the insured's physician certifying loss of limbs, loss of use or eyesight. It should be submitted to the address in the upper left hand corner.

Accident Claim

• Accident Policy (Form 181)
  This form is to be completed by the insured/patient. It, along with the itemized bills and the HIPAA form should be submitted to the address in the upper left hand corner.
• HIPAA Authorization--Health Claims (Form 2118D)
  Please, sign and date and submit a copy of this form with your claim forms for release of information.

Cancer Claim

• Cancer Policy (Form 181)
  This form is to be completed by the insured/patient. It, along with the itemized bills, the pathology report for first time claims and the HIPAA forms should be submitted to the address in the upper left hand corner.
• HIPAA Authorization--Health Claims (Form 2118D)
  Please, sign and date and submit a copy of this form with your claim forms for release of information.

Critical Illness Claim

• Critical Illness Policy (Form 185) This form needs to be completed when filing for a critical illness event. Please, be sure to check the box for the type Critical Illness being claimed and list the policy number on the form. Please, include the completed HIPAA form with your claim. It should be submitted to the address listed in the upper left corner of the form.
• HIPAA Authorization--Health Claims (Form 2118D)
  Please, sign and date and submit a copy of this form with your claim forms for release of information.

Plus Care Claim

• Plus Care Policy (Form 181)
  This form is to be completed by the insured/patient. It, along with the itemized bills and the HIPAA form should be submitted to the address in the upper left hand corner.

Hospital/Surgical Claim

• Hospital/Surgical (Form 181)
  This form is to be completed by the insured/patient. It, along with the itemized bills and the HIPAA form should be submitted to the address in the upper left hand corner.

Major Medical Claim

• Major Medical (Form 181)
  This form is to be completed by the insured/patient. It, along with the itemized bills and the HIPAA form should be submitted to the address in the upper left hand corner.

Disability Claim
