From time to time, the status of a provider may change, new providers may be added and others may leave the network or decline to accept new patients. We strongly recommend that you call the network provider to verify the provider’s continued participation in the plan. If you have any questions pertaining to network providers, please call the number on the back of your ID card.
If your plan uses the SmileMax network, the Careington network or the EyeMed network and your provider is not a member of that network, you can nominate your provider for membership.
Simply click the appropriate nomination form, print and complete it, and then submit it per instructions indicated on the form. Your provider will be invited to participate in the network.
